Cardholder application form
There are several ways to apply for a Carer Business Discount Card. The quickest way is to apply online.
Alternatively, download the cardholder application form (PDF), complete your details and return to Card Services.
If you prefer, you can make your application over the phone by calling Smart Service Queensland on 13 13 04 (Monday to Friday, 8.00 am – 6.00 pm). Remember to have your Centrelink Customer Reference Number (CRN) handy.
Who can apply?
To be eligible for a Carer Business Discount Card you must:
- be a resident of Queensland with a Queensland residential address registered with Centrelink, and
- receive the Centrelink Carer Payment or Carer Allowance.

What if my circumstances change?
You must notify Smart Service Queensland on 13 13 04 (Monday to Friday from 8.00am to 6.00pm) of any changes to your circumstances as soon as possible, including:
- changes to your name or address
- if you no longer receive the Centrelink Carer Payment or Carer Allowance
- loss of card.
Any changes to your name or address must be registered with Centrelink first. Your card eligibility is linked to the personal details registered with Centrelink and will be confirmed by the Carer Business Discount Card scheme.
Once you have contacted Centrelink with any changes, you can manage your card online (e.g. request a replacement card, update your details, or cancel a card).
Cardholder agreement
- I agree that the information I have provided is true and correct.
- I understand that the Department of Communities will use my details held by Centrelink to verify or confirm my eligibility.
- I will advise the Department of Communities of any changes in my circumstances that may affect my eligibility to hold a card.
- I understand that if I am also in possession of a Seniors Card or Seniors Business Discount Card, the Department of Communities will automatically update my current details for this scheme.
- I understand that my personal details are managed by the Department of Communities and are protected by the Department of Communities’ Privacy Plan.
- I understand the Department of Communities in partnership with Disability Services Queensland or their authorised agent/s (e.g. University) may use my details to contact me to undertake research, evaluation or review (e.g. voluntary survey/questionnaire) to ensure continuous improvement in the Carer Business Discount Card scheme.
- I understand that my personal information will not be disclosed to any other third party without my consent, unless required by law or for any other authorised purposes described in Information Standard 42 - Information Privacy.
- I understand that I can revoke this consent at any time by writing to the Department of Communities.
