Manage your card
There are several ways to manage your card. The quickest way to update your details, cancel a card, or request a replacement card is online.
Alternatively, download the cardholder application form, complete the relevant sections and return to Card Services.
If you prefer, you can call Smart Service Queensland on 13 13 04 (Monday to Friday, 8.00 am to 6.00 pm).
Your card is valid for four years and we will remind you in writing approximately six weeks before the expiry date to renew your card.
The carer named on the card is the only person who can use the card to obtain a discount or other offer. Misuse of your Carer Business Discount Card will result in cancellation of the card and further action may also be taken.
Update my information
If you have changed your residential or postal address, email address, phone number or preference to receive direct mail, you must advise us of this change.
Replace my card
If your card has been lost, stolen or damaged, please advise us so we can issue you with a new card.
Cancel my card
If you need to cancel your card for any reason (e.g. moving interstate or overseas, no longer receiving Centrelink Carer Payment or Carer Allowance, or death of a cardholder), you must advise us as soon as possible.
