Apply for a card

There are three ways you can apply for a Seniors Card or a Seniors Business Discount Card.

Replace a card

If your card has been lost, stolen or damaged, you can replace a card by completing the form online or phone Smart Service Queensland on 13 13 04 during business hours to request a replacement card.

Change card details

If you have changed your postal address, email address or phone number, change card details by completing the form online or call Smart Service Queensland on 13 13 04 during business hours to update your contact details.

If you have changed your name you need to provide us with supporting documentation, such as a photocopy of a marriage certificate, driver's licence or divorce decree. To be issued with a new card, simply send a photocopy of one of these documents, along with your old card, to the to:

Upgrade a Seniors Business Discount Card to a Seniors Card

If you are aged:

You can upgrade a Seniors Business Discount Card to a Seniors Card by completing the form online or call Smart Service Queensland on 13 13 04 during business hours.

Cancel a card

If you need to cancel an existing card for any reason (for example, you have moved interstate or the card holder is deceased) you can cancel a card by completing the form online or call Smart Service Queensland on 13 13 04 during business hours.
